Community Liaison – Business Development Manager

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Enhance and fortify referral networks.

• Generate new member leads and assist with intake conversions across IDD, Pediatric PDN, and Personal Care service lines.

• Identify, nurture, and sustain relationships with support coordination agencies, educational institutions, and community partners.

• Arrange and execute in-person visits, presentations, and follow-ups with partner organizations.

• Participate in and organize community events to represent Abound and elevate brand awareness.

• Generate new client referrals and warm leads; ensure timely communication and transfer to intake and care coordination teams.

• Monitor referral activity, conversion rates, outcomes, and trends utilizing OnTarget, Microsoft Dynamics, and Power BI.

• Offer continuous support to referral partners, including performance updates, member outcomes, and resolution of any concerns.

• Ensure adherence to ethical referral practices, healthcare regulations, and internal policies.

• Report directly to the Chief Growth Officer.

⛳️ Requirements

• Proven experience in business development, outreach, referral relations, or community engagement, ideally within healthcare, human services, or IDD/PDN programs.

• Direct experience in the PA IDD, Pediatric PDN, or Personal Care communities is highly preferred.

• Exceptional relationship-building, communication, and presentation abilities.

• Capability to analyze referral data, monitor KPIs, and report on performance trends.

• Familiarity with CRM or case management systems, including Microsoft Dynamics or similar platforms.

• Highly organized with the skill to manage multiple contacts, visits, and deadlines.

• A valid driver's license and readiness to travel locally for community visits and events.

• Bachelor's degree in Business, Marketing, Human Services, Healthcare Administration, or a related field is preferred.
