Community Health Worker, Social Impacts Team

This is a fully remote position, open to applicants in Colorado, +7 more states.

📋 Description

• Act as a liaison/connector between patients, care teams, and the community.

• Facilitate discussions with patients to eliminate obstacles to health and social services.

• Perform telephonic outreach to patients for scheduling appointments, assessing needs, and closing care gaps.

• Recognize social determinants of health that impact patients’ well-being.

• Collaborate with social workers and/or care managers to formulate action plans.

• Establish trusting relationships through patient support and advocacy.

• Assist patients in accessing health-related services and community resources.

• Accompany patients to specialist appointments and aid with enrollment forms.

• Enhance communication among patients, families, colleagues, and community-based organizations.

• Record patient interactions in medical records.

• Participate in required meetings.

• Travel to worksites and other locations as needed.

⛳️ Requirements

• High School diploma or equivalent.

• At least 1 year of experience in a healthcare, community-based, or social work setting.

• Proficiency in utilizing electronic medical records.

• Any combination of education and experience that provides a comparable background.

• Capability to work an 8-hour shift between 8am and 6pm Pacific or Mountain Time.

• Must be able to commute to the office unless accommodations are made.

• Bilingual candidates are preferred.

• Certification as a Community Health Worker is preferred.

• Willingness to travel to the worksite and other locations as needed.

• COVID-19 and Influenza vaccinations may be mandated for specific patient/member-facing positions.
