Community Health Worker – Program Manager

This is a fully remote position, open to applicants in California.

📋 Description

• Supervise, guide, and assess a team of CalAIM Community Health Workers engaged in outreach, health navigation, group facilitation, and advocacy for at-risk populations.

• Ensure adherence to CalAIM guidelines, review CPT coding (98960, 98961, 98962, T1028), implement daily/period service unit limits, and maintain boundaries concerning non-covered services.

• Manage dual-system documentation utilizing the Partnership Health Portal and Pacific Health Group EHR.

• Review member Plans of Care, conduct six-month progress updates, and standardize assessments such as SDOH 5-Q, PHQ-9/2, and Hunger Vital Sign.

• Organize necessary onboarding and ongoing education, including Motivational Interviewing, Mandated Reporting, and Stanford University Advocacy Training for Community Health Workers.

• Collaborate with RNs, licensed clinical providers, and community organizations in Humboldt County to enhance care coordination and interdisciplinary case management.

⛳️ Requirements

• Bachelor's degree in Public Health, Social Work, Healthcare Administration, or a similar field is preferred.

• A minimum of 3–5 years of experience in community health, Medi-Cal services, or social work is required.

• At least 2 years of experience in a supervisory or team leadership role.

• Strong knowledge of CalAIM policies, social determinants of health, chronic disease management, and local resources within San Diego County.

• Valid California Driver’s License (Class C) with access to a functioning vehicle.

• Clean motor vehicle record (MVR).

• Active auto insurance coverage.

• CHW certification is preferred.
