Community Health Worker

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Assist in the management of care administration.

• Offer non-clinical assistance in evaluating and assessing the needs and requirements of members.

• Direct members and their families to suitable resources for care and wellbeing.

• Support interactions with community resources.

• Make informed decisions regarding personal work methods and priorities while adhering to established directions and documented procedures.

• Occasionally travel to Humana offices for training or meetings.

⛳️ Requirements

• Must be a resident of Ohio.

• At least two (2) years of experience with community resources, health agencies, or social services.

• Intermediate skills in Microsoft Office applications (Teams, Excel, PowerPoint, Outlook, Word).

• Exceptional communication and interpersonal abilities for establishing rapport with customers and stakeholders.

• Capability to manage multiple tasks in a fast-paced environment.

• Appreciation for cultural and demographic diversity.

• Strong writing skills and advocacy for members across all levels of care.

• A separate room with a locked door designated as a home office to maintain continuous privacy is required.

• Must undergo tuberculosis (TB) screening if selected.

• A valid state driver’s license is necessary.

• Personal vehicle liability insurance that meets applicable minimum requirements is required.

• Home internet service with a minimum download speed of 25 Mbps and upload speed of 10 Mbps is needed.

• A dedicated workspace free from ongoing interruptions is essential to protect member PHI/HIPAA information.
