This is a fully remote position, open to applicants in Illinois.
• Conduct in-home evaluations, focused visits, and video consultations.
• Achieve operational, clinical, and member experience KPIs set by Signify Health.
• Evaluate new diagnostic and preventive services along with clinical workflow modifications, providing constructive feedback.
• Ensure 40 hours of weekly availability, achieve a monthly capacity of over 128 units, and dedicate one day each week for coaching and mentorship.
• Maintain a pace of 88 or more completed visits each month.
• Mentor new clinicians during their initial 90 days.
• Organize virtual and in-person shadowing sessions.
• Offer best-practice recommendations for clinical workflows and diagnostic and preventive services.
• Assist active clinicians through webinars, office hours, and real-time field support.
• Address non-urgent clinical questions and escalate any issues to Clinical Leaders or Operations Managers.
• Act as an early adopter for new services and workflow modifications.
• Participate in regular meetings with Clinical Leaders and engage in manager, team, and clinical discussions.
• Provide mentoring and coaching during assigned evening and weekend rotations.
• Ensure compliance with required training, affiliations, supplies, and diagnostic tools.
• Travel regionally and locally as business needs arise.
• Support field observations and ride-along experiences.
• A clinical background and state licensure, such as MD/DO or NP/PA, are mandatory.
• Capability to sit, stand, bend, reach, push, pull, and lift supplies weighing up to 20 pounds.
• Ability to inspire, motivate, and guide peers.
• Capacity to share best practices and firsthand experiences.
• Proficiency in managing inquiries and escalating performance concerns to leadership.
• Commitment to maintaining high-quality member care across teams.
• Experience conducting in-home evaluations in line with operational, clinical, and member experience KPIs.
• Proven track record in mentoring teams and providing personalized professional and personal support.
• Familiarity with Signify Health IHE tools and processes.
• Experience in developing and delivering group presentations.
• Multi-state licensure is preferred.
• 5 to 7 years of clinical experience, ideally in primary care, chronic disease management, or a related area.
• Minimum of 6 months experience performing in-home health evaluations for Signify Health is preferred.
• Must be available for 40 hours weekly, including assigned rotating evenings and weekends.
