Clinical Team Lead, Nurse Practitioner

This is a fully remote position, open to applicants in Texas.

📋 Description

• Direct and manage the clinical outreach program aimed at supporting complex patients.

• Lead and assist a team of nurses in coordinating care for patients.

• Develop, create, and evaluate care plans for patients with complex needs.

• Supervise nurse outreach efforts directed at complex patients.

• Address clinical escalations and manage intricate patient situations.

• Collaborate with nurses, care coordinators, specialists, pharmacists, and other healthcare professionals to enhance patient outcomes.

• Work alongside the Chief Medical Officer and other clinical personnel to refine and improve clinical operations.

• Collaborate with the Director of Care Coordination to ensure effective onboarding and continuous development of team members.

• Advocate for, support, and inspire members in managing their health and healthcare.

• Foster positive working relationships with members, families, providers, and external stakeholders.

• Operate effectively in a multicultural environment serving diverse populations.

• Establish trust, actively listen, communicate purposefully, collaborate, solve problems, and influence favorable outcomes.

• Remain seated for extended periods.


⛳️ Requirements

• Valid, unrestricted Nurse Practitioner (NP) license in Texas; must be able to obtain additional state licenses if required.

• Current and full board certification in a clinical specialty area from ANCC, AANPCB, or another recognized certifying body.

• Graduate from a professional nursing school accredited by the relevant accrediting agency, with accreditation from ACEN or CCNE.

• Over 5 years of progressive leadership experience in managing a team.

• At least 2 years of experience working remotely.

• Proficient in Chronic Care Management.

• Dedicated home office setup for remote work.

• Experience working with Veterans and/or Medicaid and/or Dual Eligible patients.

• Excellent interpersonal, verbal and written communication, and problem-solving abilities.

• Experience interacting with customers via phone and text messaging.

• Familiarity with documenting case notes in a care management or electronic health record system.

• Knowledge of motivational interviewing techniques.

• Additional licensure in Louisiana, Mississippi, Arkansas, Oklahoma, Missouri, Kansas, Nebraska, Utah, Wyoming, or Colorado is preferred.

• Preferred provider types include Adult Nurse Practitioner, Family Nurse Practitioner, Psychiatric Mental Health Nurse Practitioner (PMHNP), or Clinical Nurse Specialist (CNS).

• Experience with Apple computers (e.g., MacBook) is preferred.

• Bilingual proficiency in Spanish is preferred.
