This is a fully remote position, open to applicants in North Carolina.
• Deliver comprehensive, hands-on assistance to both patients and physicians to enhance the quality of patient care.
• Cultivate personalized relationships with patients and prescribers to boost medication adherence and advocate for evidence-based care.
• Examine clinician notes and assessments for the clinical data required for prior authorization approvals and care plan recommendations.
• Create and refine disease- and drug-specific clinical protocols.
• Track patient adherence through PDC reports, patient assessment data, and mobile platform data.
• Assess EMR information including drug/disease interactions, medication selection, labs/vitals, allergies, prescriptions, renewals, and physician communications.
• Suggest alternative therapeutic regimens and disease-specific treatments in line with established guidelines.
• Monitor therapies for side effects and medication-related concerns, recommending changes to prescribers as necessary.
• Execute medication therapy management, document interventions, and evaluate adherence rates.
• Conduct pharmacist consultations, motivational interviewing/coaching, drug regimen optimization, medication reconciliation, medication synchronization, and related clinical interventions.
• Coordinate home nurse visits and manage transitions between liaison/PSC teams.
• Track productivity and report weekly growth-development statistics.
• Supervise technicians' daily activities and verify delivery coordination slips, prior authorizations, logs, and care-utilization management.
• Identify and address insurance issues while recognizing opportunities for financial assistance.
• Oversee communications among support personnel to ensure compliance with HIPAA regulations.
• Maintain current registration and remote pharmacist activity to adhere to state and federal drug laws and DEA regulations.
• Engage directly with other health professionals regarding patient care.
• Train pharmacy staff, students, interns, externs, and healthcare professionals.
• Manage, organize, and update data using database applications.
• Support operational improvements, productivity, quality, customer-service standards, research projects, peer review, and process-improvement initiatives.
• Identify trends, resolve issues, recommend enhancements, and implement changes.
• Safeguard confidential information and uphold professional knowledge through continued education and publications.
• Perform other duties as assigned.
• The candidate must possess a Doctor of Pharmacy (PharmD) or an equivalent advanced degree.
• The candidate must be a licensed Pharmacist.
• A North Carolina pharmacist license is mandatory for this position.
• Any additional required licensure must be acquired within a mutually agreed timeframe if necessary; continued employment is contingent upon fulfilling these requirements.
• A minimum of 1 year of experience is preferred but not required.
• Excellent verbal and written communication skills are essential.
• Comprehensive knowledge of specialty medication pharmacology is required.
• Familiarity with local, state, and federal pharmacy regulations is necessary.
• The ability to manage processes while demonstrating strong technical and clinical skills is important.
• Proficiency in computer applications including Microsoft Word, Excel, PowerPoint, Outlook, and Teams is required.
• A working knowledge of database and reporting tools is necessary.
• Capability to identify barriers to medication adherence and implement as well as monitor improvement plans is essential.
• The ability to motivate and coach patients to enhance outcomes and identify therapy goals is important.
• Preferred knowledge of complex/chronic disease states and specialty pharmacy therapies.
• Strong computer skills, including internet navigation and web-based platform use.
• Data entry and computer skills will be assessed during the pre-employment evaluation.
