Clinical Pathway Navigation Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Direct the creation, execution, standardization, and continuous enhancement of clinical pathway and navigation programs across various affiliates and disease categories on an enterprise-wide scale.

• Design and provide educational and training initiatives for clinical navigation personnel regarding PCA, BCA, BPH, OAB, and other sanctioned clinical pathways.

• Offer leadership and supervision to nurse navigators and other members of the clinical team.

• Ensure adherence to established pathway standards, protocols, and best practices.

• Collaborate with physicians, nursing leaders, pharmacy teams, and operational stakeholders to incorporate clinical pathways into everyday practice.

• Act as the subject matter expert on clinical pathways and navigation programs.

• Provide guidance and consultation to affiliated practices and multidisciplinary teams.

• Monitor, gather, and evaluate metrics related to pathway adherence, key performance indicators, and patient outcomes.

• Identify challenges to pathway adoption and formulate corrective action plans and process enhancements.

• Maintain and refresh clinical pathway content, educational resources, and implementation tools.

• Prepare and deliver reports, dashboards, and program updates to executive leadership and key stakeholders.

• Collaborate with clinical and operational leadership to foster quality improvement initiatives.

• Promote teamwork among multidisciplinary teams and affiliates.

• Engage with external partners, industry organizations, and professional societies to stay informed about emerging trends and best practices.

• Advocate for resources, technology, and support to maintain and expand navigation and clinical pathway programs.

• Uphold patient confidentiality and report any suspected compliance issues or violations.

• Complete annual compliance training.

• Perform other job-related duties as assigned.

⛳️ Requirements

• RN licensure is mandatory.

• BLS certification is required.

• Graduation from an accredited nursing program is essential.

• BSN is preferred.

• Familiarity with PCA, BCA, BPH, and OAB management is necessary.

• Demonstrated experience in program coordination, education, and quality improvement initiatives.

• Exceptional leadership, communication, and interpersonal abilities.

• Capability to analyze data and convert findings into actionable improvements.

• Knowledge of clinical pathway development and implementation processes is beneficial.

• Strong verbal and written communication skills are essential.

• Excellent organizational skills and meticulous attention to detail are required.

• Robust problem-solving skills.

• Proficiency in Microsoft Office and other software applications.

• Understanding of healthcare systems operations, including EMR.

• Ability to mentor and enhance skills within the team.

• Registered Nurse with clinical experience in uro-oncology and/or oncology is preferred.

• Willingness to travel periodically.

• Ability to adhere to relevant laws, regulations, HIPAA, the Solaris Health Code of Conduct, and organizational policies and procedures.
