This is a fully remote position, open to applicants in Arizona, +9 more states.
• Interact with potential clients and document all communications in the Client Relationship Management (CRM) system.
• Conduct 60–100 outbound calls each day to prospective clients.
• Inform potential clients about Enhanced Care Management (ECM) services.
• Evaluate client needs and connect them with the care management team.
• Relay important information and client preferences to the care team for tailored care planning.
• Support clients with psychosocial evaluations and create care plans with specific, actionable objectives.
• Examine medical records to record client needs and suggest suitable interventions.
• Utilize Motivational Interviewing, Harm Reduction, and Trauma-Informed Care methodologies.
• Attend weekly Team Operations Meetings to enhance and coordinate care delivery.
• Operate remotely and/or in-person with Vynca patients and clients.
• Utilize computers, laptops, electronic health records, and video-conferencing technology during the workday.
• Availability to work Monday through Friday, from 8:30am to 5:00pm Pacific Time, with potential flexibility for evenings and weekends.
• Valid California Licensed Vocational Nurse (LVN) license, or Medical Assistant certificate from an accredited organization, or a Bachelor’s degree in a related field.
• At least 2 years of experience in telephonic outreach or enrollment within a high call volume setting.
• Background in working with complex care clients and ECM Populations of Focus.
• Strong oral and written communication abilities.
• Positive interpersonal skills for effective client interaction.
• Proficient with technology, including video conferencing, Google Workspace, Slack, and electronic health records.
• Previous experience in a remote position.
• A quiet and secure workspace to maintain member privacy.
• Capability to test internet speed to ensure smooth application functionality.
• Bilingual in English and Spanish is highly preferred.
• Candidates must reside in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, or Washington.
• Background checks, which may include drug testing or other health screenings, are required prior to employment.
• Employees who interact with patients, clients, or customers must be vaccinated against COVID-19 and influenza; accommodations may be considered.

• Ability to occasionally reach, stoop, bend, kneel, and lift items weighing up to 25 pounds.
