This is a fully remote position, open to applicants in United States.
• Deliver patient-focused Medication Therapy Management services aimed at optimizing medication therapy and enhancing clinical, economic, and humanistic outcomes.
• Conduct Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, Cost-Effective Interventions, and manage Transitions in Care.
• Engage in proactive outreach to patients to explain the benefits of the MTM program and encourage participation from hesitant individuals.
• Receive incoming calls from patients and respond appropriately in accordance with established protocols.
• Ensure accurate documentation of all discussions with patients.
• Communicate and document clinical recommendations to prescribers and interdisciplinary care teams effectively.
• Provide education centered on chronic diseases and preventive and therapeutic medication regimens to patients.
• Navigate through MTM platforms and various clinical databases or applications.
• Generate precise, concise, and timely clinical documentation.
• Offer MTM services via telephone or through remote video links (telehealth).
• Address basic technical issues related to internet connectivity, software navigation, and peripheral devices.
• At least 1 year of relevant work experience.
• Bachelor's Degree in Pharmacy or PharmD from an ACPE-accredited institution, or a graduate of an international pharmacy college.
• Valid pharmacy license in the state(s) of practice.
• Capability to obtain pharmacist licensure in multiple states upon request.
• Completion of an MTM certification course.
• Maintain active pharmacist licensure in necessary state(s) based on business requirements.
• Comply with state pharmacy regulations, CMS program mandates, HIPAA standards, and internal privacy and compliance policies.
• Proficient in evaluating medication therapy for appropriateness, effectiveness, safety, dosing accuracy, unnecessary or missing therapies, adverse drug reactions, and adherence challenges.
• Skilled in crafting evidence-based interventions.
• Familiarity with cost-effective medication strategies.
• Experience in identifying adherence barriers and devising patient-specific solutions.
• Clinical expertise in therapeutic management for adult and geriatric populations.
• Proficient in Microsoft Office applications such as Excel, Word, and Outlook.
• Ability to resolve basic technical issues independently.
• Capable of working autonomously within a collaborative environment.
• Preferred: Board Certification in a relevant specialty.
• Preferred: Completion of an ASHP-accredited pharmacy residency and/or fellowship.
• Preferred: Pharmacist licensure across multiple states.
• Bilingual proficiency in English and Spanish/Mandarin/another language is highly preferred.
