Clinical Manager, RN – Home Health

This is a fully remote position, open to applicants in Florida.

📋 Description

• Assess new referrals, verify necessary disciplines, synchronize care plans and objectives, and refresh alerts/journal notes.

• Monitor cases through audits and case discussions; remain accessible during operating hours for clinician inquiries.

• Perform ride-alongs and field coaching as clinically appropriate, including for onboarding, performance improvement, or quality enhancement initiatives.

• Eliminate obstacles to timely documentation, uphold SOPs, and ensure OASIS and care plans accurately reflect the current status.

• Encourage suitable use of Telehealth and the Patient Portal.

• Conduct regular one-on-one meetings with assigned staff and the Clinical Assistant; reinforce standards, implement corrective actions when necessary, and provide ongoing competency support.

• Execute quarterly record evaluations; share insights and recommendations with DCS/ADCS; assist in branch quality improvement tasks as directed.

• Aid in screening/interviewing candidates, suggest hires, and carry out onboarding checklists along with early-tenure coaching.

• Lead in-service sessions and training assigned by DCS; track the completion and retention of essential practices.

• Exemplify the mission and values; foster a respectful, compliant atmosphere with patients, colleagues, vendors, and leadership.

• Undertake additional responsibilities assigned by the DCS/ADCS/Regional Leadership to address patient care and operational requirements.

⛳️ Requirements

• Must possess a valid RN license (specific to the state).

• At least two years of experience in home health or a related field.

• Strong leadership and management capabilities.

• Exceptional communication, organizational, and time management skills.

• Understanding of healthcare regulations and home health care standards.

• Proficient in using electronic medical records (EMR) and other healthcare technologies.
