This is a fully remote position, open to applicants in Arizona, +10 more states.
• Initiate outreach to newly referred clients and document activities in VyncaOS.
• Conduct 8–12 intake visits each day, ensuring assessments are both efficient and precise.
• Support clients during initial intake visits, which include psychosocial assessments and care plans focused on care objectives.
• Review medical records and record client medical needs along with recommended interventions.
• Relay essential information and client preferences to the care team for tailored care planning.
• Implement evidence-based practices such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
• Participate in weekly Team Operations Meetings.
• Keep an organized schedule to maximize client intake and engagement.
• Conduct proactive outreach to clients who missed appointments or have not been contacted previously during unscheduled times.
• Be available for warm transfers from field teams and provide same-day responses.
• Achieve key performance metrics in a dynamic work environment.
• Active, unrestricted California Licensed Vocational Nurse (LVN) license, or a Bachelor’s degree in a related field, or a Medical Assistant certificate from an accredited organization (AAH, AAMA, AMT, NCCT, or NHA).
• At least 2 years of prior intake or admissions experience within a high call volume setting.
• Experience working with complex care clients and ECM Populations of Focus.
• Familiarity with Health Gorilla or Health Record Systems.
• Excellent oral and written communication abilities.
• Strong interpersonal skills and a collaborative mindset.
• Proficient computer skills, including expertise in Google Workspace, MS Office, and internet tools.
• Ability to work autonomously while maintaining high motivation levels.
• Skilled at managing multiple tasks and navigating various platforms, with the ability to adapt and shift priorities as necessary.
• Bilingual proficiency in English and Spanish is highly preferred.
• Availability to work Monday through Friday, from 8:30 AM to 5 PM Pacific Time, with potential for evenings and weekends.
• Applicants must reside in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, Utah, or Washington.
• A background check, which may include a drug test or other health screenings, is required prior to employment.
• Employees in patient, client, or customer-facing positions must be vaccinated against influenza.
• Identity and work eligibility verification through E-Verify is required upon hiring.
• Occasional travel may be necessary.
• Required physical capabilities include frequently sitting, standing, walking, talking, listening, observing, performing computer work, and utilizing laptop/video-conferencing technology; occasionally reaching, stooping, bending, kneeling, and lifting items weighing 25 pounds or less.
