This is a fully remote position, open to applicants in United States.
• Conduct Medication Therapy Management (MTM) and Stars Adherence initiatives independently within established protocols.
• Engage in proactive patient outreach utilizing advanced communication techniques.
• Coordinate patient care activities to ensure timely access to medications and continuity of therapy.
• Prioritize tasks and resolve routine issues with minimal supervision.
• Collaborate with CEC team members to maintain consistent, high-quality clinical services and shared accountability for outcomes.
• Educate patients regarding the benefits of the program and the importance of medication adherence.
• Utilize professional judgment to identify risks, address barriers, and determine suitable interventions.
• Facilitate prescription renewals and requests for extended day supplies.
• Manage exceptions and coordinate with pharmacies or providers as required.
• Recognize medication-access barriers and connect members with the appropriate clinical or operational resources.
• Escalate situations that require additional guidance, including complex, sensitive, or high-risk decisions.
• Accurately and efficiently complete assigned workflows and support overall departmental performance.
• Perform additional duties as assigned.
• A High School diploma or GED is mandatory.
• Certification as a Pharmacy Technician is required.
• A State Pharmacy Technician license is necessary where applicable.
• A minimum of 3 years of pharmacy support experience or clinical program execution is required.
• Strong pharmaceutical knowledge is essential to accurately address inquiries, support medication-related processes, and ensure safe, informed outcomes.
• Proven ability to provide empathetic, patient-focused support by actively listening, addressing needs, and ensuring clear, respectful communication across diverse populations.
• Demonstrated experience in effectively managing both inbound and outbound communications while adhering to established workflows, scripts, and escalation protocols to ensure quality and efficiency.
• Knowledge of healthcare regulations, including HIPAA, to protect patient information and ensure that all interactions and documentation comply with standards.
• Proven ability to collaborate within cross-functional teams to meet service level expectations, support shared goals, and contribute to overall operational success.
• Participation in, adherence to, and support for compliance program objectives is required.
