Clinical Case Manager, Behavioral Health, Spanish Speaking

This is a fully remote position, open to applicants in Utah.

📋 Description

• Deliver telephonic case management for Medicare beneficiaries with behavioral health requirements

• Conduct thorough assessments and formulate personalized care plans

• Employ Motivational Interviewing techniques to engage members and foster positive health outcomes

• Coordinate care with healthcare providers, health plan associates, and community resources

• Assist members in obtaining behavioral health, social, and community support services

• Offer behavioral health consultation and collaborate with internal teams

• Engage in clinical treatment rounds and interdisciplinary care discussions

• Share innovative ideas to enhance program development and continuous improvement

• Ensure precise documentation within clinical systems

⛳️ Requirements

• Active, unrestricted behavioral health license in your state of residence (LCSW, LPC, LMFT, LPCC, LISW, LSW, or equivalent)

• Master's Degree in Social Work or Counseling

• Over 3 years of recent direct clinical behavioral health experience

• Proficient in crisis intervention and motivational interviewing

• Bilingual in Spanish and English – fluent in reading, writing, and speaking

• Strong skills in assessment, care planning, and member engagement

• Capability to work independently in a telephonic, computer-based setting

• High-speed residential internet (minimum 25 Mbps download / 3 Mbps upload)
