Clinical Case Manager – Behavioral Health

This is a fully remote position, open to applicants in Iowa, +4 more states.

📋 Description

• Perform thorough behavioral health evaluations utilizing clinical instruments, data analysis, and reviews of benefit plans.

• Create personalized care strategies that address both behavioral and physical health requirements.

• Facilitate care coordination with healthcare providers, community agencies, and support services.

• Employ advanced clinical judgment to recognize and reduce risk factors that affect member outcomes.

• Execute crisis interventions for behavioral health or medical emergencies, ensuring proper referrals and follow-up.

• Utilize Motivational Interviewing methods to engage members and encourage lasting behavioral change.

• Organize psychosocial wraparound services and link members to community resources.

• Promote cost-effective care through proactive planning and efficient use of resources.

⛳️ Requirements

• Master's degree in Behavioral Health, Social Work, Counseling, Psychology, or a related discipline.

• Active, unrestricted independent behavioral health license in your state of residence (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC, or equivalent).

• Over 3 years of direct clinical experience post-master's in settings such as hospitals, ambulatory care, or outpatient clinics.

• More than 3 years of experience in treating mental health and substance use disorders.

• At least 3 years of experience applying Motivational Interviewing techniques.

• Experience in Case Management.

• Capability to converse on the phone while documenting in clinical systems simultaneously.

• Strong technical skills and comfort in a virtual work environment.
