Clinical Case Manager – Behavioral Health

This is a fully remote position, open to applicants in Iowa, +4 more states.

📋 Description

• Perform thorough behavioral health evaluations utilizing clinical instruments, data interpretation, and benefit plan assessments.

• Create tailored care plans that address both behavioral and physical health requirements.

• Facilitate care coordination with healthcare providers, community organizations, and support services.

• Use advanced clinical judgment to detect and address risk factors that may affect member outcomes.

• Execute crisis intervention for members facing behavioral health or medical emergencies, ensuring proper referrals and follow-up.

• Implement Motivational Interviewing strategies to engage members and encourage lasting behavior changes.

• Organize psychosocial wraparound services and link members with community resources.

• Promote cost-effective care through proactive planning and efficient resource utilization.

⛳️ Requirements

• A Master’s degree in Behavioral Health, Social Work, Counseling, Psychology, or a similar discipline.

• An active, unrestricted independent behavioral health license in your state of residence (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC, or equivalent).

• A minimum of 3 years of direct clinical experience post-master's in a hospital, outpatient, ambulatory, or comparable clinical setting.

• At least 3 years of experience in treating mental health and substance use disorders.

• More than 3 years of experience applying Motivational Interviewing techniques.

• Experience in case management.

• Capability to converse on the phone while documenting in clinical systems simultaneously.

• Strong technical skills and comfort in a virtual work environment.

• Experience in crisis intervention.

• Background in managed care.

• Familiarity with Utilization Review or Utilization Management.

• Experience in discharge planning.

• Experience with complex behavioral health populations.
