This is a fully remote position, open to applicants in United States.
• Oversee and coordinate behavioral health services
• Perform holistic assessments and identify challenges
• Create and modify personalized care plans
• Manage behavioral health services, care transitions, crisis prevention strategies, and aftercare
• Collaborate with members, providers, and health plan medical management teams
• Establish trusting relationships through motivational interviewing, education, and continuous communication
• Act as the primary point of contact for members
• Facilitate information sharing, discharge planning, and connections to behavioral health, community, and support resources
• Ensure timely and accurate documentation and service authorization
• Implement medical necessity criteria, regulatory requirements, accreditation standards, and internal policies
• Engage in audits, quality improvement projects, case presentations, and professional development activities
• Valid, unrestricted state license to practice independently as a Clinical Social Worker, Marriage and Family Therapist, Professional Counselor, Clinical Psychologist, or Registered Nurse in the relevant state(s)
• 3-5 years of hands-on clinical care experience in facility-based and/or outpatient behavioral health or substance use treatment environments
• Strong clinical judgment paired with effective case management abilities
• Capacity to assist members during complex care transitions
• Exceptional verbal and written communication skills
• Proficient in Microsoft Outlook and Word
• Ability to document in real-time while interacting with members
• Competence in interpreting and applying medical policies and clinical criteria
• Must pass background checks upon hiring and throughout employment
• High-speed cable or fiber-optic internet connection set up before starting, with minimum 20 Mbps download, 5 Mbps upload, and maximum 100 ms latency
• Capability to work from a designated fixed workspace at home
• Hardwired internet connection required
• Ability to sit for prolonged periods and use wired headsets and desktop equipment
• A quiet workspace with minimal background noise for calls
