This is a fully remote position, open to applicants in United States.
• Engage in patient outreach and communication via video messaging, video consultations, phone calls, and text messaging.
• Educate patients and facilitate their enrollment in the medication adherence program.
• Provide tailored education, coaching, and support for adherence using video communication.
• Offer compassionate motivational coaching, case management, and health education.
• Conduct video consultations for medication reconciliation.
• Review recorded videos of patients taking their medication to confirm administration, track adherence, and identify any side effects or concerns.
• Develop short educational and motivational videos.
• Evaluate medical, behavioral, and social needs of patients, escalating cases as needed.
• Offer guidance on medication administration, inhaler usage, chronic disease management, and medication safety.
• Serve as a clinical resource for non-clinical team members.
• Assist patients with technical issues related to the mobile application and adherence platform.
• Ensure accurate documentation in line with company and regulatory standards.
• Liaise with provider offices, health plan services, and pharmacies on behalf of patients.
• Provide referrals to healthcare providers and community resources.
• Utilize motivational interviewing techniques to encourage long-term adherence.
• Contribute insights from patient experiences to enhance program effectiveness.
• Collaborate with cross-functional teams to create video content, clinical protocols, and educational resources.
• Must be a legal resident of Puerto Rico and maintain residency during employment.
• Must be legally authorized to work in Puerto Rico/United States without employer sponsorship; visa sponsorship is not available.
• Active Registered Nurse (RN) license in good standing.
• A minimum of 5 years of nursing experience.
• At least 5 years of experience in patient engagement, health education, or care coordination.
• Fluency in both English and Spanish, including reading, writing, speaking, and comprehension.
• Strong expertise in chronic disease management, medication adherence, and patient coaching.
• Excellent verbal and written communication skills with a warm and empathetic demeanor.
• Ability to thrive in a fast-paced, patient-centered environment.
• Proficient in electronic health records (EHR), mobile applications, and digital health platforms.
• Comfortable interacting with patients via phone, video, and chat during the workday.
• Experience in assessing social determinants of health and connecting patients to resources.
• Preferred: experience in video communication, telehealth, community health work, or digital health environments.
• Preferred: health coaching certification or a willingness to obtain certification within a specified timeframe.
• Preferred: understanding of Directly Observed Therapy (DOT) models.
• Preferred: experience with Medicaid and Medicare Dual Eligible (D-SNP) populations.
• Preferred: familiarity with Salesforce or other CRM platforms.
