This is a fully remote position, open to applicants in Alabama, +43 more states.
• Adhere to the established care plan and directives of the practice.
• Deliver preventive measures and interventions for various diseases through motivational coaching.
• Foster positive relationships with patients on behalf of the practices.
• Enhance revenue by generating billable Care Management episodes and increasing visits focused on chronic condition management.
• Create comprehensive care plans for healthcare providers and patients aimed at prevention and intervention.
• Comprehend the healthcare objectives related to chronic disease management as directed by the practice.
• Oversee a designated patient panel of at least 30–50 individuals per month within the first three months, contingent on patient availability.
• Conduct thorough evaluations on 100% of assigned patients.
• Achieve billable encounters for 90% of assigned patients monthly, unless health conditions prevent participation.
• Perform chart reviews, contact attempts, calls, care coordination, documentation, and billing procedures.
• Participate in regular meetings, including Monthly Clinical Update Meetings and individual monthly meetings with a supervisor.
• Graduates from accredited Nursing Schools (LPN, LVN, RN, BSN, etc.).
• Active COMPACT license to practice as an RN/LVN/LPN in the current state of residence with no noted disciplinary actions, or licensed in the non-compact state where the practice is located.
• At least two (2) years of clinical experience in a clinic environment, Med/Surg, Case Management, or home health care.
• Practical experience with Electronic Medical Records.
• Proficiency in Windows desktop and applications, including Microsoft Office 365, Teams, and Excel.
• HIPAA-compliant workspace at home to perform Care Management responsibilities.
• Strong initiative, judgment, organizational, time-management, problem-solving, and decision-making abilities.
• Capacity to rapidly learn new technologies through video tutorials and self-guided resources.
• High-speed internet connection with a minimum of 18 Mbps download and 6 Mbps upload speeds.
• A desktop or laptop computer in good working condition, operating on Windows or Mac; Chromebooks, iPads, or tablets are not acceptable.
• Exceptional verbal, written, and listening communication skills.
• Ability to identify condition-related warning signs.
• Organized and meticulous documentation capabilities.
• Self-motivated; capable of prioritizing tasks and demonstrating effective time-management skills.
• Clear speech and outstanding phone etiquette.

• Proficiency in troubleshooting minor technical issues related to a remote work environment.
