Central Intake – Case Management Coordinator

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Oversee the intake, referral, and admission procedures for designated skilled nursing facilities

• Act as a point of contact among referral sources, hospitals, managed care organizations, residents, families, and facility teams

• Receive, assess, and process referrals

• Track referral activities and maintain comprehensive referral tracking systems and databases

• Coordinate the intake process from referral to admission

• Evaluate clinical, financial, insurance, and referral documentation

• Assess referrals for the appropriate level of care and collaborate with clinical teams regarding admission appropriateness

• Assist in transition-of-care efforts and initiatives aimed at reducing readmissions

• Communicate effectively with families and responsible parties

• Aid facilities in meeting census goals while monitoring admission activities and conversion rates

• Verify payer source details and manage authorization requirements and approvals

• Deliver excellent customer service and cultivate relationships with hospitals and community partners

• Collaborate with admissions, clinical, business office, and operational staff

• Maintain referral tracking reports and metrics related to admissions

• Ensure adherence to federal, state, and company regulations, including HIPAA

• Safeguard resident confidentiality, privacy, rights, dignity, and personal choice

⛳️ Requirements

• An Associate's Degree is required

• A Bachelor's Degree is preferred

• Knowledge of Medicare, Medicaid, and Managed Care processes

• Excellent organizational, communication, and problem-solving abilities

• Preferred experience in healthcare admissions, case management, discharge planning, or long-term care
