Case Manager – Virtual Intensive Outpatient Program, IOP

This is a fully remote position, open to applicants in Montana.

📋 Description

• Evaluate and assess adults to determine their need and suitability for care management services.

• Assist clients with the entry process into services and track their progress throughout the care continuum.

• Create, coordinate, and oversee Individual Treatment Plans (ITPs).

• Finalize treatment plans within 5 encounters or 21 days, whichever occurs first, and conduct annual updates of clinical assessments.

• Organize treatment team meetings and ensure that treatment plans focus on outcomes and recovery.

• Provide education to clients on recovery indicators and include these in the evaluation of treatment plans.

• Assess clients for Medicaid eligibility and assist with the enrollment process as necessary.

• Complete all required documentation, reports, and applications within set deadlines.

• Finish SDMI designation and Level of Impairment (LOI) assessments before beginning case management for clients with mental health diagnoses.

• Collaborate with both internal team members and external community service providers.

• Meet bi-weekly with the Program Supervisor to discuss client cases and service delivery.

• Participate in mandatory trainings, workshops, and conferences, and share key insights with team members.

• Provide services mainly through the agency's virtual Intensive Outpatient Program (IOP) telehealth platform.

• Maintain telehealth documentation that reflects clinical suitability for remote service delivery.

• Offer Community-Based Psychiatric Rehabilitation Services (CBPRS) when appropriate.

• Conduct CBPRS skills training, which includes description, modeling, practice, and performance feedback.

• Consult with family members or key support individuals as clinically appropriate.

• Deliver individual or group CBPRS while adhering to specified duration and group-size limitations.

• Document the rationale for telehealth CBPRS services and efforts to identify local support options when relevant.

• Carry out additional responsibilities as assigned.

⛳️ Requirements

• Bachelor's degree in Social Work, Psychology, or a related human services field from an accredited institution; or an equivalent combination of education and a minimum of two years of full-time experience working with adults facing mental health challenges, substance use disorders, or similar human services populations.

• Personal experience as a consumer, provider, or advocate of mental health services may be considered, with documentation of equivalency per agency policy.

• A dependable internet connection and a private, confidential workspace that is appropriate for telehealth service delivery.

• Knowledge of Montana Medicaid behavioral health regulations, SDMI criteria, Level of Impairment (LOI) documentation requirements, and targeted case management standards.

• Ability to assess and meet the clinical and functional needs of adults with substance use disorders (SUD) and/or serious mental illness.

• Familiarity with community resources and the capacity to connect clients with medical, social, vocational, recreational, or other support services.

• Strong verbal and written communication skills essential for building trust in therapeutic relationships and producing compliant clinical documentation.

• Capacity to work collaboratively within a multidisciplinary treatment team and with external community providers.

• Flexible scheduling, including potential evening hours, to accommodate client needs.

• Attendance at mandatory staff meetings, trainings, workshops, and conferences is required.
