Case Manager, RN - Field

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Oversee all case management operations with members to assess their medical requirements.

• Promote the overall wellness of members.

• Create strategies to tackle issues, outcomes, and opportunities for enhancing members' wellness through integration.

• Educate on programs and procedures in alignment with network management and clinical coverage policies.

• Travel up to 125 miles round trip for a single day in the field.

• Maintain a flexible schedule, including working 2 days per week until 9pm local time.

• Meet performance metrics and uphold compliant documentation.

⛳️ Requirements

• Must reside in Louisiana within one of the following parishes: Caddo, Bossier, Webster, Claiborne, Bienville, Red River, DeSoto, Sabine, or Natchitoches.

• Current or active clinical licensure (RN, LCSW, LPC, LMFT).

• Experience in case management.

• Strong orientation towards customer service and problem-solving.

• Excellent communication and telephonic skills.

• Proficient in motivational interviewing and building rapport and trust over the phone.

• High standards in documentation and adherence to compliance and regulatory requirements.

• Highly organized and self-motivated without the need for direct supervision.

• Effective time management skills.

• Capability to meet performance metrics.

• Proficiency with technology.

• Ability to travel within 125 miles round trip for a single day in the field.

• Flexible work schedule with the capacity to work 2 days per week until 9pm local time.

• Bachelor's and/or Master's Degree.

• CCM Certification.

• Prior experience in a Managed Care Organization.
