Case Manager, RN

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversee the comprehensive interdisciplinary care plan for patients from their admission to discharge.

• Monitor patient care and serve as a communication link between patients/families, healthcare providers, and insurance companies.

• Assess patient needs and available resources; propose and implement the optimal care plan for the best outcomes.

• Create discharge plans utilizing available resources and promoting adherence to medical recommendations.

• Identify patient care challenges and propose modifications or new clinical pathways to enhance care quality.

• Recognize patients' risk factors, barriers to care, and risks associated with discharge/readmission.

• Review care plans through chart assessments and patient discussions.

• Work collaboratively with the medical team to ensure efficient patient transitions within the healthcare system.

• Provide education to patients and families regarding the care process.

• Advocate for high-quality care and medically necessary services.

• Manage ongoing insurance authorizations for extended stays.

• Ensure compliance with regulatory notifications and patient signature requirements according to policy.

• Stay updated on insurance reimbursement guidelines.

• Design and implement discharge and transition plans that address patients’ physical, social, and emotional needs.

• Follow University and unit-level policies and procedures while protecting University resources.

⛳️ Requirements

• Bachelor’s Degree.

• Registered Nurse License (RN).

• At least 2 years of relevant experience.

• Strong oral and written communication skills.

• Capability to identify, analyze, and resolve a variety of issues.

• Proficiency in analyzing, organizing, and prioritizing tasks under pressure while meeting deadlines.

• Ability to maintain effective interpersonal relationships.
