Case Manager, RN

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversee the comprehensive interdisciplinary care plan for patients from the moment of admission until discharge.

• Supervise patient care and act as a bridge between patients/families, healthcare professionals, and insurance providers.

• Assess patient requirements and available resources, recommending and facilitating appropriate care plans.

• Create discharge strategies utilizing available resources to address ongoing patient needs and promote adherence to medical recommendations.

• Recognize patient care challenges and propose modifications or new clinical pathways to enhance care quality.

• Identify patients' risk factors, barriers to care, and potential for discharge and readmission risks.

• Review care plans through chart evaluations and patient discussions.

• Work in conjunction with the medical team to support patient transitions within the healthcare system.

• Provide education to patients and their families regarding the care process.

• Advocate for high-quality care and medically appropriate services.

• Manage ongoing insurance authorizations for extended stays.

• Handle regulatory notifications and ensure patient signatures are collected in accordance with policy.

• Stay informed about insurance reimbursement policies.

• Plan and implement discharge and transition strategies that cater to patients' physical, social, and emotional needs.

• Comply with University and unit-level policies and procedures while protecting University assets.

⛳️ Requirements

• Bachelor’s Degree.

• Registered Nurse (RN) License.

• At least 2 years of relevant experience.

• Strong ability to communicate effectively in both verbal and written formats.

• Proficient in recognizing, analyzing, and resolving a variety of issues.

• Capable of analyzing, organizing, and prioritizing tasks under pressure while adhering to deadlines.

• Ability to maintain productive interpersonal relationships.
