This is a fully remote position, open to applicants in New Jersey.
• Create proactive care plans that address identified issues and improve both short- and long-term outcomes.
• Assess, plan, implement, and coordinate activities related to case management.
• Evaluate the medical needs of members and promote overall wellness.
• Utilize clinical tools and data analysis to assess member needs and available benefits.
• Employ clinical judgment to mitigate risk factors and barriers while addressing intricate health and social indicators.
• Perform holistic assessments using claims data and other information sources, including co-morbid conditions and multiple diagnoses.
• Determine the necessity for referrals to clinical resources and interdisciplinary team members.
• Collaborate with supervisors, healthcare stakeholders, members, providers, and community organizations.
• Present cases in interdisciplinary case conferences.
• Adhere to regulatory and company policies and procedures related to case management.
• Utilize motivational interviewing techniques to engage members and assess their health status and needs.
• Meet members in person within the designated Middlesex County area.
• A minimum of 3+ years of clinical practice experience is required.
• Active and unrestricted RN licensure in New Jersey.
• Willingness and capability to travel 25–50% using personal vehicle for face-to-face member meetings.
• Reliable transportation is essential.
• Must live near or within Middlesex County, New Jersey.
• A dedicated workspace free from interruptions is required.
• Arrangements for dependent care during work hours must be separate.
• Certification as a Case Manager is preferred.
• Preferred experience of 2+ years in Care Management, Discharge Planning, and/or Home Health Care Coordination.
• Strong analytical and problem-solving abilities are essential.
• Effective communication, organizational, and interpersonal skills are necessary.
• Ability to work independently is required.
• Competent computer skills, including the ability to navigate multiple systems and proficient keyboarding.
• Proficiency in MS Word, Excel, Outlook, PowerPoint, and proprietary applications is expected.
• An Associate's Degree is mandatory.
• A direct/hardwired internet connection must be available within 7 feet of the computer.
• Minimum internet speed of 25 Mbps download and 3 Mbps upload is required.
• A quiet, secure, and private designated home workspace that complies with CVS Health and HIPAA guidelines is necessary.
• Must work within the same state and city where you currently reside.
• Provision of personal workspace furniture is required.
• Ability to utilize multiple monitors and applications simultaneously is necessary.
• Proficiency in using email, calendar invites, Teams messaging, and virtual meetings is required.
• Experience with Microsoft Office 365 or similar Google Workspace applications is preferred.
• Ability to access secure systems such as VPN and EHR portals is required.
• Must adhere to computer and password security practices and recognize suspicious emails or links.
• Ability to troubleshoot common technical issues independently is necessary.
• Bilingual candidates are preferred.
• NJC Certification is preferred but not mandatory.
