Case Manager, Registered Nurse – Field

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Collaborate and contribute to the case management process by utilizing critical thinking and sound judgment.

• Facilitate optimal healthcare outcomes for members.

• Assist in scheduling appointments.

• Identify and support members in accessing their benefits and educational resources.

• Utilize care management tools and resources effectively.

• Serve dual-eligible Medicare and Medicaid populations facing complex health and social challenges.

• Partner with members, providers, and community organizations.

• Address the healthcare and social determinant needs of members.

• Travel 50–75% of the time from the home location within Cook County and its surrounding areas.

• Operate independently from a designated home workspace while collaborating with teams in a virtual environment.

• Navigate various systems and applications, including secure VPN and EHR portals.

• Communicate through email, calendar invitations, Teams messaging, and virtual meetings.

• Resolve common technical issues and reach out to IT when necessary.

⛳️ Requirements

• Must reside in the state of Illinois.

• Must live in or near Cook County and its surrounding areas in Illinois.

• Must have reliable transportation and be willing to travel up to 50–75% of the time from home within Cook County and the surrounding areas in Illinois.

• A minimum of 3–5 years of clinical practical experience is required.

• Must be confident in working from home as an independent thinker, utilizing tools for virtual collaboration with teams.

• Excellent analytical and problem-solving capabilities are essential.

• Strong communication, organizational, and interpersonal skills are necessary.

• Ability to work independently is a must.

• Proficient computer skills, including the ability to navigate multiple systems and keyboarding skills.

• Proficiency in standard corporate software applications, such as MS Word, Excel, Outlook, and PowerPoint, as well as proprietary applications.

• An Associate's degree is required.

• Active and unencumbered Registered Nurse license in the state of Illinois is mandatory.

• A direct/hardwired internet connection to a modem/router within 7 feet of the computer, with a minimum of 25 Mbps download and 3 Mbps upload speeds; WiFi and satellite internet are not permitted.

• A quiet, secure, and private designated home workspace that complies with CVS Health and HIPAA guidelines.

• Must work within the state and city of current residence.

• Must provide own workspace furniture.

• Required to meet internet speed standards and adhere to Work From Home guidelines.

• Comfortable setting up and utilizing multiple monitors while navigating several applications simultaneously.

• Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings.

• Experience with Microsoft Office 365 applications or similar Google Workspace tools is necessary.

• Ability to access secure systems such as VPN and EHR portals, manage strong passwords, and identify suspicious emails or links.

• Competence in independently resolving common technical issues and contacting IT for unresolved matters.
