Case Manager, Registered Nurse – Field

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Apply critical thinking and judgment to support and enhance the case management process.

• Promote optimal healthcare outcomes for our members.

• Assist in scheduling appointments.

• Identify members' needs and help them access their benefits and educational resources.

• Utilize care management tools and resources effectively.

• Work collaboratively with members, healthcare providers, and community organizations.

• Address the healthcare and social determinants of members' needs throughout the continuum of care.

• Travel as required throughout Sangamon, Macon, Logan, and nearby areas.

⛳️ Requirements

• Must be a resident of Illinois.

• Must have reliable transportation and be willing to travel up to 75% of the time from home within Sangamon, Macon, and Logan counties.

• A minimum of 3-5 years of clinical practical experience is required.

• A dedicated workspace that is free from interruptions is necessary.

• Dependents must have alternative care arrangements during work hours.

• Must be confident working independently from home, utilizing tools to collaborate and connect with teams virtually.

• Strong analytical and problem-solving abilities are essential.

• Effective communication, organizational, and interpersonal skills are required.

• Ability to work autonomously is important.

• Proficient computer skills, including the ability to navigate multiple systems and effective keyboarding.

• Proficient in MS Word, Excel, Outlook, PowerPoint, and proprietary applications.

• An Associate's degree is mandatory.

• An active and unencumbered Registered Nurse license in Illinois is required.
