Case Manager, Registered Nurse

This is a fully remote position, open to applicants in Texas.

📋 Description

• Perform thorough telephonic evaluations and create tailored care plans.

• Recognize health risks, obstacles, and opportunities to enhance member wellness.

• Manage care, referrals, and transitions to suitable clinical programs and resources.

• Utilize clinical judgment to assist members with complex and co-existing conditions.

• Work collaboratively with multidisciplinary teams to achieve the best possible outcomes for members.

• Ensure adherence to regulatory standards and company policies.

• Offer telephonic case management support for members facing intricate healthcare challenges.

• Aid members in navigating resources, enhancing health outcomes, and addressing barriers to care.

⛳️ Requirements

• Current, unrestricted RN license in the state of residence.

• Openness to obtaining a Compact RN license (company-funded).

• At least 5 years of clinical RN experience.

• Associate Degree in Nursing (ADN).

• Capability to work during standard business hours, Monday through Friday.

• A minimum of 6 months of experience in Case Management or Utilization Management (preferred).

• Case Management Certification (CCM or equivalent) (preferred).

• Bachelor of Science in Nursing (BSN) (preferred).
