Case Manager – Mental Health Pathways, Family Support

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Deliver mental health rehabilitation services and support to adults and families.

• Reduce hospitalizations and crisis situations while enhancing community functioning.

• Assist clients in accomplishing their rehabilitative, resilience, and recovery objectives.

• Ensure continuity of program services in a safe, ethical, legal, moral, and therapeutic manner.

• Collaborate as a team member in providing mental health services to clients and their families.

• Address recovery challenges in clients’ daily living circumstances.

• Complete necessary documentation within specified timelines.

• Assume primary case management duties for assigned clients.

• Conduct individual, group, and family counseling, as well as educational sessions.

• Maintain client service records in accordance with regulations.

• Communicate effectively with admissions, process, and discharge planning personnel.

• Coordinate services with community resources.

• Apply treatment concepts, evidence-based practices, and various service modalities.

• Present workshops, lectures, and training sessions to clients, families, staff, and community organizations.

• Be knowledgeable about court proceedings and represent Rosecrance clients in court as needed.

• Engage in Clinical Team meetings and activities.

• Uphold confidentiality according to the professional Code of Conduct and relevant laws.

• Provide outstanding customer service and positively represent the agency.

• Take on additional responsibilities as assigned by management.

⛳️ Requirements

• Bachelor’s degree in Psychology, Counseling, Social Work, or a related field, or equivalent experience and training.

• Valid driver’s license and ability to fulfill Agency insurance criteria.

• Must possess personal transportation.

• Self-motivated individual capable of working independently as well as part of a team.

• Proficiency in computer usage is essential.

• Ability to navigate and document in electronic healthcare records is required.

• Competence in accurately completing required documentation within established timeframes.

• Availability to work Monday through Friday, from 8:30 AM to 5:00 PM.

• Must successfully pass an occupational health screening.

• Must operate in a zero-tolerance drug-free environment, which includes marijuana.
