Case Manager – Mental Health Pathways

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Deliver mental health rehabilitation services and support to adults and families.

• Reduce hospitalization and crisis incidents while enhancing community functionality.

• Assist clients in reaching their rehabilitative, resiliency, and recovery objectives.

• Ensure continuity of program services in a secure, therapeutic, ethical, legal, and moral fashion.

• Collaborate as a team member in providing mental health services to clients and their families.

• Address recovery matters in clients’ daily living environments.

• Assume primary case management duties for assigned clients.

• Conduct individual, group, and family counseling, along with educational sessions.

• Keep client service records in accordance with regulations.

• Engage with admissions, process, and discharge planning personnel.

• Coordinate services from community resources.

• Implement treatment concepts, evidence-based practices, and service modalities.

• Facilitate workshops, lectures, and training for clients, families, staff, and community groups.

• Attend court as required on behalf of Rosecrance clients.

• Contribute to Clinical Team meetings and activities.

• Uphold confidentiality in line with the professional Code of Conduct and relevant laws.

• Provide outstanding customer service and positively represent the agency.

• Execute other related tasks as assigned by management.

⛳️ Requirements

• Bachelor's degree in Psychology, Counseling, Social Work, or a related field, or equivalent experience and training.

• Valid driver's license and the ability to meet Agency insurance criteria.

• Must have personal transportation.

• Self-motivated with the capacity to work independently as well as collaboratively.

• Proficiency in computer usage is essential.

• Ability to navigate and document within electronic healthcare records is required.

• Competence in completing necessary documentation accurately and within specified timeframes.

• Capability to motivate and address challenges regarding effective clinical practices.

• Ability to tailor verbal and written communication to suit the language and style of clients and their families.

• Familiarity with court procedures and availability to make court appearances for Rosecrance clients.

• Must pass an occupational health screening.

• Availability to work Monday through Friday, from 8:30 AM to 5:00 PM.
