Case Manager – Mental Health Pathways

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Deliver mental health rehabilitation services and support to individuals and families.

• Reduce instances of hospitalization and crises while enhancing community functioning.

• Provide program services in an ethical, legal, and moral manner, ensuring a safe therapeutic environment.

• Actively engage as a team member in the provision of mental health services.

• Integrate recovery topics into clients' everyday living circumstances.

• Inspire staff and address challenges related to effective clinical practices.

• Complete all necessary documentation within specified timeframes.

• Take on primary case management duties for assigned clients.

• Facilitate individual, group, and family counseling as well as educational sessions.

• Keep client service records compliant with relevant regulations.

• Liaise with admissions, process, and discharge planning teams.

• Coordinate services from community resources efficiently.

• Employ treatment concepts, evidence-based practices, and various service modalities.

• Conduct workshops, lectures, and training sessions for clients, families, staff, and community organizations.

• Represent Rosecrance clients in court appearances as necessary.

• Participate in Clinical Team meetings and related activities.

• Uphold confidentiality in accordance with the professional Code of Conduct and applicable laws.

• Provide outstanding customer service and positively represent the agency.

• Undertake additional responsibilities as assigned by management.

⛳️ Requirements

• Bachelor’s degree in Psychology, Counseling, Social Work, or a related field, or equivalent experience and training.

• Must possess a valid driver’s license and meet Agency insurance criteria.

• Personal transportation is required.

• Self-motivated individual with the ability to work both independently and collaboratively.

• Proficiency in computer usage is essential.

• Capability to navigate and document within electronic healthcare records is required.

• Must accurately complete required documentation within designated timeframes.

• Availability to work Monday through Friday, from 8:30 AM to 5:00 PM.

• Successful completion of an occupational health screening is necessary.

• Commitment to a zero-tolerance drug-free workplace, including marijuana.
