Case Manager – Mental Health Pathways

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Provide rehabilitation services and support for mental health to adults and families.

• Reduce instances of hospitalization and crises while enhancing community engagement.

• Offer mental health services in a manner that is ethical, legal, moral, safe, and therapeutic.

• Act as a member of the clinical team to serve clients and their families.

• Assume primary case management duties for assigned clients.

• Facilitate individual, group, and family counseling sessions, as well as educational workshops.

• Maintain client service documentation in compliance with established regulations.

• Coordinate communication with staff involved in admissions, treatment processes, and discharge planning.

• Organize services from community resources.

• Implement treatment concepts, evidence-based practices, and various service modalities.

• Conduct workshops, lectures, and training sessions for clients, families, staff, and community members.

• Attend court hearings and represent clients in legal proceedings as necessary.

• Engage in clinical team meetings and activities.

• Uphold confidentiality in accordance with the professional Code of Conduct and relevant laws.

• Provide outstanding customer service and positively represent the agency.

• Carry out additional responsibilities as assigned by management.

• Bachelor’s degree in Psychology, Counseling, Social Work, or a related field, or equivalent experience and training.

• Possess a valid driver’s license and meet the Agency's insurance criteria.

• Must have reliable transportation.

• A self-motivated individual capable of working independently as well as collaboratively within a team.

• Proficiency in computer usage is essential.

• Ability to navigate and document within electronic healthcare records is required.

• Aptitude for accurately completing necessary documentation within specified timeframes.

• Knowledge of court procedures and willingness to appear in court for Rosecrance clients.

• Must pass an occupational health screening.

• Compliance with a zero-tolerance drug-free workplace policy, including marijuana, is required.
