This is a fully remote position, open to applicants in Arizona, +15 more states.
• Facilitate coordination of care across inpatient, outpatient, emergency room, and home-care environments while upholding confidentiality and professional standards.
• Collaborate with members and providers to navigate intricate health issues and develop customized care plans.
• Evaluate and address Social Determinants of Health by linking members with community support services.
• Provide crisis intervention and advocate for members to eliminate obstacles to care and ensure treatment compliance.
• Interact with members via phone, secure messaging, or in-person to educate them about health conditions, hospital discharges, medical procedures, and medications.
• Adhere to relevant laws and regulations.
• Participate in initiatives for process enhancement and new pilot programs.
• Report directly to the Manager of Clinical Programs.
• Undertake additional responsibilities as assigned.
• A minimum of 2 years of clinical experience, including hospital, outpatient, or community-based care management.
• At least 2 years of experience working directly with patients suffering from chronic medical conditions such as diabetes, COPD, or hypertension.
• A minimum of 1 year of experience in Care Coordination and Navigation.
• Active, unrestricted RN licensure from the United States in [state], OR an active compact multistate unrestricted RN license, OR a Master of Social Work (MSW) from a nationally accredited school recognized by the Council on Social Work Education.
• Candidates must be residents of Arizona, Florida, Georgia, Illinois, Iowa, Kansas, Michigan, Missouri, Nebraska, New Jersey, North Carolina, Ohio, Pennsylvania, South Carolina, Tennessee, Texas, or Virginia.
• Bonus: Bachelor of Science in Nursing (BSN).
