This is a fully remote position, open to applicants in Arizona, +9 more states.
• Evaluate the needs of members across various aspects including physical health, mental health, substance use disorders, oral health, palliative care, memory care, trauma-informed care, social support, housing, and community services.
• Supervise the creation of client care plans and set achievable goals.
• Provide services in-office, via telehealth, or in the field where members can easily access care.
• Connect clients with essential social services and support systems.
• Advocate on behalf of clients with healthcare professionals.
• Implement evidence-based practices such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
• Conduct outreach and engagement to promote linkage to the Enhanced Care Management program and document activities in the CRM system.
• Assess client progress and adjust SMART goals accordingly.
• Facilitate mental health promotion initiatives.
• Organize transportation for clients.
• Ensure the completion of documentation and outcome measures within the specified timeframes of care plans.
• Maintain up-to-date patient health records in the EMR and other business systems.
• Prepare monthly reports to ensure program compliance.
• Participate in assigned training sessions.
• Coordinate person-centered care in collaboration with healthcare providers, specialists, pharmacists, social service agencies, caregivers, family supports, and other stakeholders on the care team.
• Manage healthcare benefits and ensure timely, cost-effective access to care.
• Encourage wellness, recovery, independence, resilience, and empowerment among members.
• A minimum of 2 years of experience as a care manager, care navigator, or community health worker assisting vulnerable populations.
• Willingness and ability to work Monday to Friday, from 8:30 AM to 5:00 PM Pacific Time, with flexibility for potential evenings and weekends.
• Familiarity with government and community resources related to social determinants of health.
• Clean driving record.
• Valid driver's license.
• Reliable means of transportation.
• Strong oral and written communication skills.
• Excellent interpersonal skills.
• Basic computer proficiency.
• Proficient in Google Workspace, MS Office, and navigating the internet.
• Bilingual in English and Spanish is preferred.
• Must be legally authorized to work in the United States.
• Completion of background screening, which may include a drug test or other health assessments, is required.
