This is a fully remote position, open to applicants in Arizona, +9 more states.
• Act as the main coordinator for clients with intricate requirements.
• Collaborate with healthcare providers, specialists, pharmacists, social service organizations, caregivers, family support individuals, and other members of the care team.
• Oversee client cases and manage healthcare benefits.
• Deliver education and ensure timely, cost-effective access to care.
• Encourage wellness, recovery, independence, resilience, and empowerment among members.
• Evaluate member needs in areas such as physical health, mental health, substance use disorders, oral health, palliative care, memory care, trauma-informed care, social support, housing, and community services.
• Supervise the creation of client care plans and objectives.
• Provide services in-office, through telehealth, or in the field where members are located, seek care, or can access services most conveniently.
• Connect clients with essential social services and support systems.
• Advocate for clients with healthcare practitioners.
• Employ principles of Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
• Conduct outreach and engagement activities, documenting these in the CRM system.
• Assess client progress and revise SMART goals accordingly.
• Facilitate mental health promotion.
• Organize transportation options, such as ACCESS.
• Complete necessary documentation and outcome measures within the designated care-plan timelines.
• Keep current patient health records in the EMR and additional business systems.
• Prepare monthly reports to ensure program compliance.
• Attend required training sessions.
• A minimum of 2 years of experience as a care manager, care navigator, or community health worker serving vulnerable populations.
• Availability to work Monday to Friday, from 8:30 AM to 5:00 PM Pacific Time, with flexibility for occasional evenings and weekends.
• Working knowledge of governmental and community resources related to social determinants of health.
• Clean driving record, valid driver's license, and dependable transportation.
• Exceptional oral and written communication abilities.
• Strong interpersonal skills.
• General computer proficiency and familiarity with Google Workspace, MS Office, and the internet.
• Bilingual in English and Spanish is preferred.
• Candidates must reside in Arizona, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, Utah, or Washington.
• A background check is necessary prior to employment, which may include drug testing or other health screenings.
• Employees in roles that interact with patients, clients, or customers must be vaccinated against influenza, with accommodations considered.
• Completion of identity and work eligibility verification using E-Verify is required upon hire.
• Occasional travel may be necessary.
• Ability to perform routine activities such as sitting, standing, walking, talking, listening, observing, working on a computer, and using laptop/video conferencing; occasional lifting of items weighing 25 pounds or less.
