This is a fully remote position, open to applicants in Pennsylvania.
• Deliver ongoing, family-focused case management to children, youth, and their caregivers.
• Involve families using trauma-informed, strengths-based, and family-centered methodologies.
• Conduct continuous assessments of family needs, strengths, safety issues, protective factors, and service obstacles.
• Create, execute, and oversee personalized service plans.
• Perform required monthly in-home visits and additional contacts based on family needs and risk levels.
• Monitor child safety, family stability, service engagement, and progress towards objectives.
• Report emerging issues through designated channels.
• Assist families in accessing behavioral health, medical, educational, substance use, parenting, housing, employment, transportation, and community resources.
• Offer practical support, coaching, and guidance in navigating resources.
• Maintain regular communication with caseworkers and supervisors from referring agencies.
• Collaborate with schools, behavioral health providers, medical professionals, courts, juvenile probation, community organizations, and other involved entities.
• Engage in staff meetings, multidisciplinary case reviews, supervisory sessions, court proceedings, and required gatherings.
• Coordinate and monitor referrals while ensuring continuity of care.
• Document case notes, assessments, service plans, contact records, court reports, weekly reports, statistical reports, and other necessary documentation.
• Ensure confidentiality of records in VisionQuest and approved systems.
• Adhere to agency protocols, state regulations, child protective services laws, mandated reporting requirements, policies, and contractual obligations.
• Communicate any suspected abuse, neglect, safety threats, critical incidents, or significant family changes per protocol.
• Participate in supervision, case consultation, quality assurance, training sessions, and continuous quality improvement efforts.
• Travel throughout the designated service area for home visits, meetings, court proceedings, and service coordination.
• Transport children, youth, or family members when authorized and necessary.
• Maintain a flexible schedule, including evenings and nontraditional hours, as required.
• Fulfill other assigned duties.
• Bachelor's or Associate's degree in behavioral sciences, human services, or social services fields is preferred.
• At least 1 year of case management or child welfare experience.
• Strongly encouraged to have child welfare and/or case management experience.
• A minimum of one year of related work experience is preferred.
• Must be at least 21 years of age.
• Flexible schedule, including evenings, weekends, and holidays.
• Successful clearance of Child Abuse and Neglect and FBI criminal background checks.
• Valid driver's license and eligibility to drive for program services.
• Strong skills in family engagement, assessment, case management, crisis response, and service coordination.
• Ability to identify and respond appropriately to concerns regarding child safety, family risk, and mandated reporting.
• Excellent written and verbal communication abilities.
• Capability to prepare clear, timely, and objective case documentation and court reports.
• Proficient in collaborating with families, agency personnel, courts, schools, service providers, and multidisciplinary teams.
• Knowledge of child welfare systems, trauma-informed care, family preservation methods, and community resources.
• Cultural humility and effectiveness in working with diverse families.
• Ability to work independently in community settings while maintaining communication with supervisors.
• Strong organizational, time-management, and computer skills.
• Ability to learn electronic case management systems.
• May need to lift and carry up to 10 pounds frequently.
• Capability to stand or walk 50% of the day and sit for the remaining 50%.
• Ability to bend, stoop, reach, and grasp as required.
