Case Manager

This is a fully remote position, open to applicants in Arizona, +5 more states.

📋 Description

• Oversee care coordination across inpatient, outpatient, emergency room, and home care environments.

• Collaborate with members and healthcare providers to address intricate health challenges and develop customized care plans.

• Evaluate and tackle Social Determinants of Health by linking members with community support services.

• Offer crisis intervention and advocate for members to eliminate obstacles to care and ensure treatment compliance.

• Contact members via phone, secure messaging, or face-to-face to provide education on health conditions, hospital discharges, medical procedures, and prescriptions.

• Uphold confidentiality and maintain high standards of professionalism.

• Adhere to relevant laws and regulations.

• Participate in process improvement initiatives and new pilot projects.

• Carry out additional responsibilities as assigned.

• Report to the Case Management supervisor.

⛳️ Requirements

• A minimum of 2 years of clinical experience in hospital, outpatient, or community-based care management.

• At least 2 years of direct experience with patients suffering from chronic medical conditions (such as diabetes, COPD, hypertension), showcasing a strong understanding of disease trajectories, standard treatment regimens, and symptom management.

• A minimum of 1 year of experience in Care Coordination and Navigation.

• Possession of an active, unrestricted RN license from the United States OR an active compact multistate unrestricted RN license or a Masters in Social Work (MSW) from a nationally accredited institution recognized by the Council on Social Work Education.

• A BSN, licensed social worker credentials, CCM Certification, case management experience, complex case management certification, and proficiency in bilingual Spanish are considered advantageous.
