This is a fully remote position, open to applicants in Wisconsin.
• Evaluate the health and safety requirements of members and formulate detailed Member Care Plans.
• Collaborate with a Field Care Nurse to deliver services in alignment with each Member Care Plan.
• Perform in-person social assessments during enrollment and at least every six months, usually at members' homes.
• Conduct quarterly face-to-face visits and maintain monthly phone communication.
• Organize support services pertaining to social integration, community resources, employment, housing, and other non-medical needs.
• Ensure the provision of cost-effective services.
• Assess risk factors and offer educational guidance to members.
• Keep accurate records of case notes, service authorizations, and updates to Member Care Plans.
• Provide services to members enrolled in the Wisconsin Family Care program in Calumet, Outagamie, and Winnebago counties.
• Travel up to 40% of the time.
• A Bachelor's degree in human services or a related field along with a minimum of 1 year of experience working with frail elders or adults with intellectual, developmental, or physical disabilities.
• Alternatively, a Bachelor's degree in a different field with at least 3 years of relevant experience.
• A valid state driver's license is required.
• Proof of personal vehicle liability insurance with minimum limits of 100/300/100.
• Preference for candidates residing within 45 minutes or less of the designated coverage area.
• Experience in Case Management is preferred.
• Familiarity with electronic case note documentation is preferred.
• Knowledge of community health and social service agencies and additional community resources is preferred.
• Availability to work Monday–Friday from 8:00 am to 4:30 pm CST.
• A minimum home internet speed of 25 Mbps download and 10 Mbps upload is required for home or hybrid home/office employees.
• Ability to work from a designated space free from ongoing interruptions to safeguard member PHI/HIPAA information.
• Completion of a TB screening is mandatory if selected.
