Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Evaluate the health and safety requirements of members while recognizing their strengths, interests, and preferences.

• Create thorough Member Care Plans (MCPs).

• Collaborate with a Field Care Nurse to meet the health and safety needs of members.

• Visit members' locations for in-person social assessments upon enrollment and at least every six months.

• Conduct quarterly home visits and maintain monthly phone communication.

• Organize support services related to social integration, community resources, employment, housing, and other non-medical necessities.

• Ensure services are delivered in a cost-effective manner.

• Assess risk factors and provide educational support to members.

• Keep precise case notes, service authorizations, and updates to MCPs.

• Support frail elders and adults with intellectual, developmental, or physical disabilities as part of the Wisconsin Family Care program.

⛳️ Requirements

• A Bachelor's degree in human services or a related field, along with a minimum of 1 year of experience working with frail elders or adults with intellectual, developmental, or physical disabilities.

• Alternatively, a Bachelor's degree in another discipline with at least 3 years of relevant experience.

• Willingness to travel up to 40% of the time to meet members.

• A valid state driver's license is required.

• Proof of personal vehicle liability insurance with minimum limits of 100/300/100.

• TB screening is mandatory for this patient-facing position.

• A dedicated workspace free from interruptions to safeguard member PHI/HIPAA information.

• Home internet service with a minimum download speed of 25 Mbps and upload speed of 10 Mbps for employees working from home or in a hybrid setup.
