Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Deliver thorough case management services to elderly individuals and adults with intellectual, developmental, or physical disabilities within the Wisconsin Family Care program.

• Evaluate members' health and safety requirements, strengths, interests, and preferences to create detailed Member Care Plans.

• Collaborate with a Field Care Nurse to meet members' health and safety needs in the least restrictive settings.

• Perform in-person social assessments upon enrollment and at least every six months, usually at members' homes.

• Conduct quarterly face-to-face visits and maintain monthly phone interactions with members.

• Organize support services focused on social integration, community resources, employment, housing, and other non-medical necessities.

• Ensure the delivery of services is cost-effective.

• Assess risk factors and provide education to members.

• Keep accurate case notes, service authorizations, and updates to Member Care Plans.

⛳️ Requirements

• A Bachelor's degree in human services or a related field, along with at least 1 year of experience working with frail elders or adults with intellectual, developmental, or physical disabilities; or a Bachelor's degree in another discipline with a minimum of 3 years of such experience.

• Ability to travel up to 40% of the time to visit members.

• Possession of a valid state driver's license.

• Proof of personal vehicle liability insurance with a minimum coverage of 100/300/100.

• Preferably reside within 45 minutes or less of the designated coverage area.

• Experience in case management is preferred.

• Familiarity with electronic case note documentation is preferred.

• Knowledge of community health and social service agencies, as well as additional community resources, is preferred.

• TB screening is required if selected.

• Access to home internet with download speeds of at least 25 Mbps and upload speeds of 10 Mbps.

• A dedicated workspace that minimizes interruptions to safeguard member PHI/HIPAA information.
