This is a fully remote position, open to applicants in Arizona, +2 more states.
• Evaluate, plan, implement, and manage case management activities for members.
• Assess members’ healthcare needs and promote overall wellness.
• Create proactive strategies to enhance both short- and long-term outcomes.
• Utilize clinical tools and data analysis to assess members’ requirements and benefits.
• Exercise clinical judgment to mitigate risk factors and address intricate health and social issues.
• Perform thorough assessments encompassing co-morbidities and various diagnoses.
• Evaluate referrals to clinical resources and interdisciplinary team members with a holistic perspective.
• Collaborate with supervisors and healthcare partners to address obstacles and achieve objectives.
• Adhere to case management procedures, regulatory standards, and company protocols.
• Implement motivational interviewing techniques to enhance member engagement and evaluate health needs.
• Advocate on behalf of clients and the providers of members.
• Available to work Monday–Friday during standard business hours in the local time zone, with one to two days per week until 7 PM MT or PST.
• Registered Nurse with an active, unrestricted nursing license holding multi-state/compact privileges.
• Capability to obtain licensure in all non-compact states.
• Must reside within the Continental United States.
• Residence in MT or PST time zones is required.
• Minimum of 3 years of clinical/nursing practice experience.
• At least 1 year of experience in medical case management, care coordination, or discharge planning.
• Over 3 years of experience with personal computers, keyboarding, navigating multiple systems, and MS Office Suite applications.
• An Associate's degree in nursing is mandatory.
• Experience in telephonic case management is preferred.
• High-speed internet with a direct/hardwired connection; a minimum of 25 Mbps download and 3 Mbps upload speed is required.
• A quiet, secure, private designated home workspace that complies with CVS Health and HIPAA standards.
• Capability to work within the state and city of residence.
• Proficiency in using multiple monitors and navigating various applications.
• Familiarity with email, calendar invitations, Teams messaging, and virtual meetings.
• Experience with Microsoft Office 365 or comparable Google Workspace applications.
• Ability to access secure systems such as VPN and EHR portals.
• Competence in managing strong passwords and identifying suspicious emails or links.
• Ability to independently troubleshoot common technical issues.
• Provision of a desk or standing desk for extended work durations.
