This is a fully remote position, open to applicants in Illinois.
• Perform thorough assessments of members’ needs and eligibility utilizing care management tools and information/data analysis.
• Suggest strategies for case resolution and addressing members’ needs by assessing benefit plans and available programs/services.
• Recognize high-risk factors and service requirements, referring members to clinical case management or crisis intervention when necessary.
• Organize and execute assigned care plan activities while monitoring the progress of the care plan.
• Collaborate with case managers, supervisors, Medical Directors, and other health programs to address obstacles.
• Present cases at case conferences for multidisciplinary assessment.
• Spot and escalate quality-of-care concerns through the appropriate channels.
• Negotiate suitable options and services to fulfill members’ benefits and healthcare requirements.
• Employ influencing and motivational interviewing techniques to engage members and encourage lifestyle/behavior modifications.
• Offer coaching, information, and support to enable members to make autonomous medical and healthy lifestyle choices.
• Participate in ongoing team meetings and provide peer mentoring/training.
• Assist members in engaging knowledgeably with providers in healthcare decision-making.
• Apply case management and quality management processes in accordance with regulatory, accreditation, and company standards.
• A minimum of 2 years of experience in Behavioral Health, Social Services, or a relevant field equivalent to program focus.
• Must be willing and able to travel 50-75% of the time within Cook County, Illinois.
• Reliable transportation is essential.
• Must possess 2+ years of experience with electronic documentation and Microsoft Office applications.
• Experience in case management and discharge planning is preferred.
• Managed Care experience is an advantage.
• A Bachelor's degree or non-licensed master's level clinician is required.
• The degree must be in Behavioral Health or Human Services (Psychology, Social Work, Marriage and Family Therapy, Counseling).
