Case Management Coordinator – Field

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Assess the needs and eligibility of members using care management tools and data review.

• Suggest strategies for case resolution and fulfilling member needs by analyzing benefit plans and available programs/services.

• Recognize high-risk factors and service requirements, directing members to clinical case management or crisis intervention when necessary.

• Coordinate and execute assigned care plan activities while monitoring progress.

• Collaborate with case managers, supervisors, Medical Directors, and other health programs to address barriers.

• Present cases during case conferences for multidisciplinary evaluation.

• Identify and escalate issues related to the quality of care.

• Negotiate suitable options and services for members’ benefits and healthcare requirements.

• Utilize influencing and motivational interviewing techniques to engage members and encourage lifestyle and behavioral changes.

• Offer coaching, information, and support for independent medical and healthy lifestyle decisions.

• Participate in team meetings and provide peer mentoring/training.

• Assist members in making informed decisions regarding their healthcare.

• Implement case management and quality management processes in adherence to regulatory, accreditation, and company standards.

⛳️ Requirements

• Minimum of 2 years of experience in Behavioral Health, Social Services, or a related field relevant to the program.

• Must be willing and able to travel 50-75% of the time in and around Chicago, Illinois.

• Reliable transportation is essential.

• At least 2 years of experience with electronic documentation.

• Proficiency in Microsoft Office applications is required.

• A Bachelor's degree or a non-licensed master's level clinician is required, with either degree in Behavioral Health or Human Services (Psychology, Social Work, Marriage and Family Therapy, Counseling).

• Standard business hours are Monday to Friday, 8:00 am to 5:00 pm CST.
