In-Home Health – Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Conduct in-home health assessments for members of health plans.

• Carry out visits lasting approximately 45 minutes, which include reviewing medications and medical history as well as performing physical evaluations.

• Execute additional diagnostic tests as requested by the member’s health plan, such as diabetic eye exams, spirometry, and bone density screenings.

• Utilize an iPad and follow clinical workflows for each evaluation.

• Establish trustworthy relationships and contribute to enhanced health outcomes.

• Travel nationwide to connect individuals with suitable care.

• Travel locally and within licensed states as required.

• Conduct virtual visits when necessary.

• Deliver Signify Health services, including Diagnostic & Preventive Services products.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Willingness to travel locally and within licensed states as needed.

• Capability to conduct visits in private residences that may have household pets, secondhand smoke, and airborne allergens or irritants.

• Ability to safely perform duties in various home environments.

• Qualification as a Nurse Practitioner or Physician Assistant.
