This is a fully remote position, open to applicants in Illinois.
β’ Collaborate and contribute to the case management process
β’ Facilitate optimal healthcare outcomes for members
β’ Assist with scheduling appointments
β’ Identify and support members in accessing benefits and educational resources
β’ Conduct thorough evaluations of member needs and eligibility
β’ Assess benefit plans along with available internal and external programs and services
β’ Identify high-risk factors and service requirements, referring to clinical case management or crisis intervention as necessary
β’ Coordinate and execute assigned care plan activities while monitoring progress
β’ Consult with case managers, supervisors, Medical Directors, and other health programs to address barriers
β’ Present cases during case conferences for multidisciplinary review
β’ Identify and escalate issues regarding quality of care
β’ Negotiate suitable options and services for members
β’ Utilize motivational interviewing techniques to engage members and encourage lifestyle or behavioral changes
β’ Provide coaching, information, and support for independent medical decisions and healthy lifestyle choices
β’ Participate in team meetings and offer peer mentoring and training
β’ Assist members in making informed healthcare decisions
β’ Employ case management and quality management processes in adherence to regulatory, accreditation, and company requirements
β’ Minimum of 2 years experience in Behavioral Health, Social Services, or a relevant related field aligned with program focus
β’ Willingness and ability to travel 50-75% of the time within Mason, Cass, Scott, and Brown counties, Illinois
β’ Reliable transportation is mandatory
β’ At least 2 years of experience in electronic documentation
β’ Proficiency in Microsoft Office applications
β’ Bachelor's degree or non-licensed master's level clinician required in Behavioral Health or Human Services (Psychology, Social Work, Marriage and Family Therapy, Counseling)
β’ Previous experience in case management and discharge planning is preferred
β’ Experience in Managed Care is preferred
