Case Management Analyst – Field

This is a fully remote position, open to applicants in Florida.

📋 Description

• Coordinate care to promote optimal health outcomes for members

• Educate members and facilitate access to suitable healthcare services

• Utilize care management tools and data analysis to evaluate member needs and identify risks

• Create and implement effective care plans

• Work collaboratively with internal teams, providers, and external resources to overcome barriers

• Enhance the quality of care and ensure services are aligned with member benefits

• Encourage member engagement, informed decision-making, and healthy behavior changes through effective communication, coaching, and motivational techniques

• Track the progress of care plans

• Record activities

• Ensure adherence to regulatory standards, company policies, and quality management guidelines

⛳️ Requirements

• Must possess reliable transportation and be willing/able to travel up to 75% of the time

• Must reside in Brandon/Riverview & Surrounding Areas, Florida

• Minimum of 1 year experience in a behavioral health or long-term care environment

• Proficient in Microsoft Office Suite and general computer skills

• Bilingual (English & Spanish): Required

• Must successfully complete both verbal and written language assessments

• Experience in discharge planning, case management, or managed care (MCO)

• Strong multitasking, prioritization, and adaptability skills in a fast-paced setting

• Advanced communication abilities in both English and Spanish (verbal and written)

• Bachelor’s Degree or non-licensed Master’s level clinician required

• Degree should be in behavioral health, human services (psychology, social work, marriage and family counseling)
