This is a fully remote position, open to applicants in North Carolina.
• Oversee ongoing care for designated individuals to ensure continuity and stability across various services and environments.
• Facilitate coordination of services among providers and community resources for effective long-term care management in a facility setting.
• Educate individuals on available community services and resources.
• Connect individuals to community services and resources to promote community-based integration.
• Involve individuals and families to encourage sustained engagement in services and care plans.
• Evaluate needs, formulate care plans, and coordinate services among providers and community resources to enhance continuity of care and achieve long-term outcomes.
• Must achieve QP status through one of the designated pathways: full licensure by the NC governing board that regulates Human Services professions; fully licensed RN eligible to practice in North Carolina with four years of full-time experience in the MH/DD/SA field; a master’s degree in a human service discipline with one year of full-time experience in MH/DD/SA or qualifying supervised substance abuse counseling experience; a bachelor’s degree in a human service field with two years of full-time experience in MH/DD/SA or qualifying supervised substance abuse counseling experience; or a bachelor’s degree in another discipline with four years of full-time experience in MH/DD/SA or qualifying supervised substance abuse counseling experience.
• If working with members requiring LTSS, a minimum of two years of prior experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management is essential.
• Must be fully vaccinated against Covid-19, influenza, MMR, varicella, and Tdap, with necessary proof of immunity documentation.
• A valid driver's license is required.
• Residence within Trillium's Mid State Region or Southern Region: Anson, Guilford, Montgomery, Randolph, Richmond, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, or Scotland counties is mandatory.
• Ability to travel within the catchment area as needed.
• Successful completion of a drug test is a prerequisite for employment.
