Care Planner – Behavioral Health

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Deliver extensive support and services to behavioral health clinical teams.

• Enhance member engagement and aid in the provision of high-quality care.

• Optimize program and operational efficiencies related to clinical matters.

• Reach out to members regarding specialty programs and encourage their participation.

• Assist members in identifying services and facilitate follow-up communications.

• Gather information on patient progress and adherence to treatment plans.

• Provide insights into mental health and substance abuse benefits, community treatment resources, managed care programs, and Aetna's policies and procedures.

• Collaborate closely with clinicians to promote positive member outcomes.

• Conduct searches for providers and community resources.

• Manage Transition of Care requests.

• Implement letters of agreement with non-participating providers.

• Secure discharge verifications and handle non-complex authorizations.

• Perform tasks that involve prolonged sitting, telephone communications, and computer typing.

⛳️ Requirements

• Two years of experience in Behavioral Health care.

• Over one year of experience in Customer Service.

• Competence in computer skills, including navigating various systems and keyboarding.

• Access to wired residential broadband service with internet speeds of at least 25 mbps/3 mbps.

• An Associate's Degree or an equivalent combination of education and experience.

• Work schedule: Monday–Friday, 8:00 am–4:30 pm in the employee's time zone.

• Bilingual skills are preferred, with Spanish being ideal, though any language will be considered.

• Previous experience in Managed Care is preferred.

• Experience in Psychology or Social Services is preferred.
