Care Navigator – Transitions of Care

This is a fully remote position, open to applicants in Florida, +3 more states.

📋 Description

• Coordinate follow-up care plan requirements by scheduling appointments or enrolling members in relevant programs.

• Evaluate member adherence to medical treatment plans via telephone or through on-site visits.

• Identify obstacles to plan adherence and facilitate resolutions.

• Recognize opportunities impacting quality objectives and suggest process enhancements.

• Propose adjustments to treatment plans and assess the necessity for additional services with case management and healthcare providers.

• Oversee the identification and referral to local, state, or federally funded programs.

• Guide members on strategies to minimize health risks.

• Prepare reports that document case progress and compliance updates.

• Build and sustain relationships with agencies specified in relevant contracts.

• Work five eight-hour shifts each week, including one Saturday and one Sunday per month.

• Adhere to Eastern Time hours of 8:00 AM–5:00 PM or Central Time hours of 7:00 AM–4:00 PM.

⛳️ Requirements

• High School diploma or equivalent.

• At least 1 year of relevant experience.

• Any combination of education and experience that would provide an equivalent background.

• Bilingual or multilingual skills may be necessary for the Carelon - CareBridge business unit.

• Certification as a nurse assistant or medical assistant is preferred.

• A BS/BA degree in a related field is preferred.

• Preference for bilingual candidates.

• Strong oral, written, and interpersonal communication abilities.

• Proficient problem-solving skills.

• Effective facilitation skills.

• Strong analytical abilities.

• Must reside within a reasonable commuting distance from one of the job posting locations unless an accommodation is provided as required by law.

• New hires in specific patient/member-facing positions must receive vaccinations against COVID-19 and Influenza unless an acceptable justification is provided.
