Care Navigator – OB

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Develop, evaluate, and coordinate care management activities tailored to member needs.

• Create or assist in the formulation of personalized care plans/service plans.

• Educate members and their families/caregivers about services, benefits, healthcare access, care options, referrals, and provider instructions.

• Assess member needs, identify obstacles to care, and evaluate available resources.

• Recognize and facilitate access to providers, specialists, community resources, and local support services.

• Offer psychosocial and resource assistance to members, caregivers, and care managers.

• Coordinate efforts among members, families/caregivers, and care provider teams to ensure timely service access.

• Track care-plan objectives and monitor member status or changes in condition.

• Collaborate with healthcare providers to update care plans and address member requirements.

• Gather, document, and maintain member data and care-management activities to ensure regulatory compliance.

• Conduct on-site visits when appropriate.

• Undertake additional duties as assigned while adhering to policies and standards.

⛳️ Requirements

• Bachelor's degree.

• 2–4 years of relevant experience.

• Alternatively, equivalent experience obtained through applicable knowledge, duties, scope, and skills that reflect the position level.

• If possessing clinical licensure, graduation from an accredited nursing school.

• Preference for a current state clinical license.

• Capability to work remotely from anywhere within Missouri.

• Availability Monday–Friday, 8 am–5 pm.

• Ability to provide education and support to pregnant members.
